- Properly implement DllCanUnloadNow ref counting. Make sure the mozilla
  control is ready to unload as well.
- Use Interlocked* instead of ++/-- in AddRef/Release.
- Use only stored result of Interlocked* in AddRef/Release.
- Expand TRACEs to display the ref count.
