- Use IoAllocateMdl instead of being a cowboy and using MmCreateMdl.
- Handle failure/out-of-memory cases in IoBuildAsyncronousFsdRequest
- Free the input buffer on failure in IoBuildDeviceIoControlRequest
- Handle failure in IoBuildSyncronousFsdREquest.
